CMS has completed a comprehensive update of the home health conditions of participation (CoPs). The updated CoPs enhances the clinicians accountability around patient’s rights, care planning and coordination, infection control, and emergency preparedness. How care is provided and documented will need to be examined and changes implemented in every home health agency in order to ensure compliancy with the new rules. This webinar is the first step in identifying specific areas, which require work flow changes by the clinician at the patient level.

Objectives

  • Understand the new patient rights element, including ways to be compliant when providing those rights to the patient/family.
  • Identify the elements required in writing to ensure effective coordination of care; visit schedule, medication schedule, treatments, pertinent instructions, and name of the clinical manager.
  • Write and incorporate patient centered goals when creating their patient plan of care.
  • Integrate the new CoP requirements to their admission, transfer, and discharge process.

Coleen Schmidt has been a leader in the home care industry for over 20 years, working the last 16 years at Horizon Home Care & Hospice, Inc. She is dedicated to a positive patient experience and is focused on service diversity, improving patient outcomes, and ensuring regulatory compliance. Coleen considers herself a life-long learner and is currently pursuing a masters in nursing, with an emphasis in education. When she's not at work or school, you can find her attempting a whole new level of multi-tasking, which involves family, exercise (mostly thinking about it), more hobbies then time, and just enjoying the moment.

Bobbi Schmidt has been in home health care for 18 years. Eleven of those years have been in home care Leadership. Bobbi is currently with Horizon Home Care and Hospice as the Director of Home Care. She has been the Board Chair for WiAHC for 4 years and previously on the WHO board for 2 years. Bobbi serves on the Education and Membership Committees for WiAHC. Bobbi received her BSN and Masters in Healthcare from the University of Phoenix. Bobbi enjoys boating in the summer as well as driving/riding the countryside in a sports car with her husband. In the winter she enjoys snowmobiling.
