CAHPS Hospice Survey

Survey Instruments:

The CAHPS Hospice Survey mail and telephone materials from the Quality Assurance Guidelines V 5.0 (QAG V5.0) in all approved translations have been posted to the CMS CAHPS Hospice Survey website, Survey Instruments webpage.  Survey vendors must utilize the revised CAHPS Hospice Survey materials beginning with January 2019 decedents (caregivers surveyed beginning April 1, 2019).

CY 2018 Participation Exemption For Size:

The Participation Exemption for Size Form for the calendar year (CY) 2018 CAHPS Hospice Survey data collection and reporting requirements is available to complete and submit on the CAHPS Hospice Survey website, Participation Exemption for Size webpage.   Hospices that served fewer than 50 survey-eligible decedents/caregivers in CY 2017 (January 1, 2017 through December 31, 2017 [or from assignment of CCN]) can apply for an exemption from participation in the CAHPS Hospice Survey for CY 2018.

The Participation Exemption for Size Form will be available to complete and submit online until December 31, 2018. NOTE:  The participation exemption for size applies only to the CAHPS Hospice Survey.  It does no tapply to the Hospice Item Set (HIS).

CAHPS Hospice Survey exemptions on the basis of size are active for one year only. If a hospice continues to meet the eligibility requirements for this exemption in subsequent years, the organization will need to again request the exemption.

If a hospice has questions regarding the Participation Exemption for Size process or how to complete the Participation Exemption for Size Form, please contact the CAHPS Hospice Survey Project Team: or 1-844-472-4621.

Preview Reports

Hospice provider preview reports and Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Survey ® provider preview reports have been updated and are now available. These two separate reports are available in your Certification and Survey Provider Enhanced Reports (CASPER) non-validation reports folder. Hospice providers are encouraged to review their Hospice Item Set (HIS) quality measure results from Quarter 2, 2017 to Quarter 1, 2018 and their facility-level CAHPS® survey results from Quarter 2, 2016 to Quarter 1, 2018.

Providers have 30-days to review their HIS and CAHPS® results (December 10, 2018 through January 9, 2019).  Should a provider believe the denominator or other HIS quality metric to be inaccurate or if there are errors within the results from the CAHPS® Survey data, a provider may request CMS review. Providers must adhere to the process outlined on the Public Reporting: HIS Preview Reports and Requests for CMS Review of HIS Data webpage and the Public Reporting: CAHPS® Preview Reports and Requests for CMS Review of CAHPS® Data webpage
