Transitions: Inside and Outside the Four Walls

This year’s WHA Post-Acute Care Conference provides an opportunity for colleagues working on patient care transitions from a variety of positions and perspectives to gather in person for the first time in several years to learn from each other and hear from a strong group of presenters. At this year’s conference, presenters will discuss leading through change, the value of Care Transitions Intervention (formerly the Coleman Model), and hospital at home care in Wisconsin. Attendees also will hear forward-looking insights from the administrator of the Wisconsin Division of Quality Assurance, from panelists who will share how hospitals and Family Care managed care organizations can work together to improve care transitions, and more.
